Please read these Terms & Conditions carefully before booking your appointment.

Appointment Scheduling and Cancellation:

  • All appointments must be confirmed at least 24 hours prior to the scheduled time through Calendly.

  • Cancellations or rescheduling requests must be made at least 24 hours before your appointment. You can cancel or reschedule your appointment via the Calendly link/email with your appointment information.

  • Late cancellations (within 12 hours) and missed appointments without notification will be charged the full appointment fee. Appointments are reserved for you specifically and cancellation within 12 hours disrupts our schedule and limits access for other patients.

  • Appointments made within 24 hours of scheduled time can be canceled up to an hour before the scheduled time. Terms and conditions to late cancellations and missed appointments apply.

Payment and Refunds:

  • Payment is due in full at the time of your appointment. We accept various payment methods electronically.

  • No refunds will be issued for appointments canceled within 12 hours or for missed appointments.

  • For appointments made within 24 hours of scheduled time, no refunds will be issued for cancellations within 1 hour or missed appointments

  • If you have extenuating circumstances, please contact us as soon as possible and we will do our best to accommodate your request.

Arrival and Punctuality:

  • Please arrive on time for your scheduled appointment time. This allows us to maximize time with you to answer any questions or concerns you may have during your session. 

  • If you arrive more than 10 minutes late, your appointment may be considered canceled and you will be charged the full fee. We recommend making arrangements with the possibility of unforeseen delays in mind.

Health Information Form: 

  • For Standard and Premium package customers scheduling an initial consultation, please complete your health information form 24 hours prior to your appointment. This allows our expert genetic counselors to review your health history and ask the appropriate questions and maximize your time and experience during your session. 

  • For the Standard package, if you are unable to complete your health history form 24 hours prior to your appointment, please be prepared to provide details during your first session. You may need to schedule a follow up session/s at an additional cost.

Confidentiality:

  • We are committed to protecting your privacy and confidentiality. All information discussed during your appointment is protected by HIPAA regulations.

Communication:

  • We encourage you to ask questions and communicate openly throughout your appointment.

  • You will receive our contact information after booking your appointment and are welcome to reach out with any questions or concerns.
